New student Group Application

PLEASE NOTE:

  • Please see the SMSS Student Group Policy found in SMSS Policies Package prior to completion of this form. Should you have any questions following reading this document, please contact the Student Groups Coordinator. In SMSS Policies package there is information on how to correctly fill out these application forms. 
  • Incomplete or altered forms will not be considered.
  • All forms should be signed by an executive member and submitted by email.
  • Please retain a copy of this form for your records.

All New student Group Applications need to be submitted by October 15th. Forms to submit to have a completed application include:

  1. Completed application Form
  2. Completed Potential Member List
